I doubt its fake, the only thing suspicious about it is that the guy is inside the car while its going on, technically it should be grounded and you should be safe, but I still wouldn't risk it, and I have a feeling most people with the know how for this wouldn't either.

 

They aren't really that hard to make though, If you read Hackaday they do specials on them all the time. I've seen some pretty crazy ones on there before. Way bigger than that one too.

 

Oh and you guys know that the end result doesn't look like that right? When people take pictures of their Tesla Coils they do it with a really long exposure time so you can see everything better. That thing is rotating around the car and is really only in one spot at once.
