Whether a thug on the streets or a big crime boss, he has never been this druggo-looking deadbeat. The Joker is a character who has been protrayed differently many times so it's cool that different actors, writers, artists create different types of the character. It did the rounds before any real pics of the Joker were available (before Ledger was definitely cast). Someone mocked this up to show what he could look like.
