The PSX BIOS should automatically detect if the inserted memory card is not formatted and ask you if you want to format the card.

You're forgetting that in ePSXe, when you run a game, the PSX Bios isn't even loaded (At least not completely). You need to use the Memcard Manager and create a memory card file first.

And if you own a real Playstation, this isn't the case. They come formatted.

I meant when you actually go into the PSX Memory Card Manager. But yeah, emulator-wise you need to manually create the memory card file first. The manager only asks you to format if the card is damaged.

I assumed (falsely) that GordoOne already had a memory card file, since ePSXe comes with two memory card files.

ePSXe can manage the memory card if you choose "Run BIOS" instead of "Run CD-ROM" but otherwise yes it will simply skip the bios and load the game. Also some playstation games will allow you to format your memory card form within the game so that you don't have to reset and use the BIOS memcard manager. A good example of this would be GT2.
